For People With Insurance
Dr. Slone accepts the following PPO insurance plans: Aetna, Anthem, Blue Cross, Blue Shield, and Cigna. Our office staff manages electronic billing directly to your insurance company. You are responsible for any co-payments and/or deductible at the time services are rendered. If you would like us to verify your insurance coverage before scheduling a visit, please complete the form below.
For all other insurance plans, Dr. Slone will work with you as an out of network provider, and we will provide a superbill that you can submit directly to your insurance company for reimbursement. Please check with your insurance company to verify your reimbursement rate.
